My friend has an old backup file of his iPhone from last year, including
20Gb or so of his music.

Unfortunately due to his silly confusion when he passed his MBP to his
daughter abroad, he failed to keep a full backup of this music and all
he has left is this backup file (she erased the music in the meantime).

Is there any way he can extract the music files from this backup ?

the backup, which is actually a folder of thousands of files rather
than a single file, does not contain music, movies or apps. it does
    have photos, however.

he will need to use a utility to copy the music off his iphone, unless
his daughter wants to send the music to him (if she hasn't erased it).
there are several such utilities. check ilounge.com.

So ... when an iPhone is synced or backed up ... it doesn't back up the
    Music at all ?
The music is no longer on his iPhone. And she has already erased it.

what's to back up? the music is already on the computer before it was
copied to the iphone, so there's no need to copy back again to the
computer. think of it as already having been backed up.
any music he purchased from the itunes music store can be redownloaded
    at no charge.

any music he encoded himself from his own cd or record collection will
    need to be done again.

The iOS backup is a backup fo all the data on the device that is not
    already in iTunes.
    Then it's gone.

If you right-click in your iPhone in iTunes, there should be a "Transfer Purchases" option. That will redownload anything of music from the iTunes Store back to your iPhone.
