Fill-Outable PDF Forms

Anyone still use this newsgroup? If so, can anyone educate me on what
system/software can be used to produce and process such forms? A
typical example is the Medicare Claim Form at
www.humanservices.gov.au/spw/customer/forms/resources/pc1-1210en.pdf.
