As mentioned elsewhere possibly a power supply problem.
I had a very similar problem with a 2 year old Power supply, it turned
out that there was too much load on the ±5V lines causing the contacts to
overheat - this resulted in visible charring (take out the motherboard
power plug and look for discoloured contacts) and resulting voltage
fluctuations due to the increased resistance.
I was getting about about +4.4 and -5.6V! I usualy sorted this out by
unplugging the power connector and rocking it side to side to help clean
the contacts, this did the trick, but it was only temporary and the
problem re-occured.
I eventually ended up buying a new Power supply (same power rating) and
haven't had it since.
